The address where you will be staying in the United States is optional. If multiple locations are planned, enter the first address. If the full address is not known, enter the name of the hotel or city and state you will visit.
I acknowledge that I am waiving, for the duration of the authorization granted to me under ESTA, any right to review or appeal a U.S. CBP officer's determination as to my admissibility, or to contest any action for deportation arising from a request for admission under the Visa Waiver Program, other than on the basis of an application for asylum.
Furthermore, as a condition of my admission to the United States under the VWP, I agree that submission of my biometric data (including fingerprints and photographs) upon arrival constitutes a reaffirmation of my waiver of any right to review or appeal.
